During your initial visit you will be seen by our staff who will ask you about your medical history and your symptoms. A focused examination will then be made by your doctor.
While many patients want to discuss in great detail the exact event which may have caused their injury, the most important information your health care team needs involves your symptoms (such as where you have pain, weakness, numbness, etc.) and what limitations these symptoms place on your every day activities.
After reviewing your medical history and diagnostic tests, you will be presented with a number of treatment options. We will inform you of the risks and benefits of each option and will assist you in making the final decision concerning which option is best for you.

Try to arrive at least 20 minutes in advance in order to complete the necessary paperwork. We will try to make your visit as convenient as possible. Due to emergencies, delays sometime occur but we make every effort to follow the appointment schedule.
Payment, including co-payment and anticipated deductibles, is expected at the time of service. If you have any questions regarding our billing procedure please call the office and we will be glad to answer your questions. Accepted forms of payment are cash, check or credit card.
Our office participates with several insurance plans. Please call your insurance plan, or the office, to verify participation.
Please let us know if you will be unable to keep your appointment. We will then be able to accommodate others and reschedule your appointment if necessary.
Request for X-rays
If you need to pick up your x-rays or have them mailed, please call the office a few days ahead of time so that these can be prepared for you. When you call please let the office know where and to whom the x-rays are to be sent. There is a $5.00 advanced payment for mailing x-rays.
Disability Forms & Release of Medical Records
Disability forms and release of medical records are handled in a timely manner. Please give the office ample time to have these requests completed. There is a $5.00 charge for completing disability forms, paid in advance. We cannot process same day requests.
Any request for release of records must be accompanied by a signed release from the patient. Copies for an attorney must be requested by the attorney along with an authorization for release of records.
